I, {name} understand and acknowledge that the activity in which I am about to voluntarily engage in bears certain known and unanticipated inherent risks that contribute to the unique character of this activity and may be hazardous to participants regardless of all feasible safety measures taken by the provider. These include, but are not limited to, my physical condition and physical exertion for which I am not prepared, dehydration, heat stroke, motion sickness, inaccessibility to immediate medical attention, and my or other participant’s acts or omissions which could result in injury, death, illness or disease, physical or mental, or damage to myself or to my property. I agree that I am the only one who can judge my condition, health and abilities and I will immediately stop my participation in or use of the activities if I should feel pain, dizziness, any discomfort or feel in any way abnormally.

I hereby accept and expressly assume all risks associated with participating in any activities and specifically release, discharge and waive any and all claims I may have, now or in the future, known or unknown against Arena Training Institute including any claims which may be made by my representative heir or assigns for any injury, loss or death. I further agree to indemnify (reimburse for any loss) and hold harmless the released parties from any loss or liability (including any reasonable attorney’s fees they may be incurred) in defending any claim made by me or anyone making a claim on my behalf or as a result of my participation in or use of any activities.

I am aware that there is no obligation for any person to provide me with medical care. If medical care is rendered I hereby release any claim against Arena Training Institute as a result of such medical care.

I grant permission for Arena Training Institute to utilize any photographs, videos or recordings
for commercial use or on use of social media.

I understand and intend that this document acts as the broadest and most inclusive assumption of risk, waiver, release of liability, agreement not to sue as permitted by the laws of the State of Wyoming.

I expressly agree and promise to accept and assume all responsibility and risk for the above
arising from my participation in this activity. My e-signature indicates that I have completely
read this document, understand its contents and am voluntarily waiving certain legal rights.
